Job Description
The Transition in Place (TIP) Case Manager will provide case management and supportive mental health and substance use counseling to a caseload of up to 16 Veterans. The role assists Veterans in their transition from homelessness to housing through permanent housing lease assumption.
Requirements
Bachelor's Degree
Two to three years' experience in case management and working with substance use disorders, mental illness and with a diverse population
Knowledge and commitment to evidence-based models such as Motivational Interviewing, Relapse Prevention and Trauma Informed Care
Massachusetts Driver's License
