Job Description
At Home Harmony is seeking experienced Nurse Practitioners or Physician Assistants to join our Post-Acute/Transitional Care Team. The ideal candidate will have a minimum of three years of experience in post-acute care and be able to provide exceptional care to complex needs patients in the comfort of their homes or via telehealth.
Requirements
Minimum of three years of experience as an APP in Post-Acute, SNF, Hospitalist, Geriatrics, Family Practice, Internal Medicine
Graduate with an advanced degree from an accredited institution
Current unrestricted VA state license as a Nurse Practitioner or Physician Assistant
Nationally board certified, current
Current BLS required
Prescriptive authority and active DEA
Valid drivers license with clean driving record
