The role integrates patient/family-centered care, interdisciplinary communication, education, regulatory compliance, and accurate documentation to optimize outcomes and support program performance.
What You Will Do
Coordinate care for transplant candidates, recipients, and living donors, manage time-sensitive workflows, and ensure accurate documentation within EMR and UNetSM modules.
