Role OverviewThe Case Manager provides comprehensive case management and coordinated supportive services to eligible veterans, surviving spouses, and dependent family members. They conduct client assessments, determine program eligibility, develop individualized service plans, and provide emergency financial assistance. The Case Manager works collaboratively with participants to promote eviction prevention, housing stability, and long-term self-sufficiency.
What You Will Do
Conduct comprehensive intake, eligibility screening, and needs assessments. Verify client eligibility, develop individualized service plans, and provide comprehensive case management. Determine eligibility for emergency financial assistance and provide referrals to internal and external resources.

Requirements
- Bachelor's degree in Social Work preferred but not required
- Equivalent experience in social work or business-related field
- Minimum of 2 years of experience in nonprofit setting with at least 1 year of experience in Social Services and/or Case Management experience within the Veteran population
- Bilingual in English and Spanish is preferred but not required
- Valid driver's license with a clean driving record
- Valid vehicle liability insurance
