Job Description
As a Waiver Case Manager, you will have the opportunity to foster trusting connections with the individuals you serve, develop comprehensive care plans, and connect individuals with community resources to increase their independence and overall well-being.

Requirements

  • One year of experience in the education or treatment of people with developmental disabilities or related conditions OR a minimum of one course that specifically focuses on developmental disabilities
  • Bachelor’s degree in social work and a current, active social work license issued by the Minnesota Board of Social Work
  • Valid driver's license, reliable vehicle for work purposes, acceptable driving record, and proof of auto insurance
  • Excellent written and verbal communication skills in English
  • Passing all background and requirements for facility and position
