Job Description
Thomas Allen, Inc. is seeking a Waiver Case Manager to develop comprehensive care plans, coordinate services, and connect individuals with community resources to increase their independence and overall well-being.

Requirements

  • Bachelor’s degree in social work and an active social work license issued by the Minnesota Board of Social Work or bachelor's degree in special education, psychology, nursing, human services or other fields related to the education or treatment of people with developmental disabilities or related conditions.
  • One year of experience in the education or treatment of people with developmental disabilities or related conditions OR a minimum of one course that specifically focuses on developmental disabilities (Note: If you do not meet this requirement, we provide training!)
  • Valid driver's license, reliable vehicle for work purposes, acceptable driving record, and proof of auto insurance.
