Role OverviewWe are seeking a dedicated and experienced Wound Nurse Coordinator to join our healthcare team. The ideal candidate will be responsible for coordinating wound care services, providing expert nursing care to patients with acute and chronic wounds, and collaborating with multidisciplinary teams to ensure optimal patient outcomes.
What You Will Do
Assess, plan, implement, and evaluate wound care treatment plans for patients. Coordinate wound care services across departments and with external providers as needed.

Requirements
- Licensed Practical Nurse (LPN) license in the state

- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Proficient in electronic medical records (EMR) and documentation
