Job Description
Child and Family Guidance Center is seeking a clinical YES Wraparound Facilitator for the North Texas (DFW) locations. The primary responsibility of the Wraparound Facilitator is to provide intensive case management services to families of children with severe emotional disturbances.

Requirements

  • Minimum 10-13 children/families caseload
  • Provide a minimum of 60 hours of services per month per family
  • Accept assigned cases, develop and maintain a team of mental health professionals and natural supports
  • Coordinate and facilitate monthly child and family team meetings
  • Provide weekly case management and care coordination
  • Maintain accurate and concise documentation
  • Maintain an empathic, yet professional, relationship with children and families
