Job Description
Youth Case Manager position at MoCo ReConnect Drop-in Center to support youth ages 16-24 with housing insecurity, alternative education pathways, or seeking a safe space. Provides referrals, support, and follow-up, and builds transforming relationships with disconnected youth.

Requirements

  • Working as part of a multidisciplinary team
  • Completing intakes and needs assessments
  • Building authentic relationships with youth
  • Coordinating with other community agencies
  • Providing a trauma-informed approach
  • Providing advocacy and crisis de-escalation
  • Maintaining contact with service providers
  • Providing flexible case management
  • Supporting youth at the MoCo ReConnect Drop-in Center
